We provide 245D Basic and Intensive supports under Minnesota CADI, BI, DD, and CAC waivers. Each plan is built collaboratively with the person, their family, and their county case manager.

Our intensive support services are intended to promote training, habilitation, or rehabilitation. We work with behavior analysts, nurses, and therapists to deliver consistent, evidence-based support.
Services for people who live in their own home or with family — providing support and training across community-living domains so people can participate fully in the life they choose.
Trusted, planned relief for primary caregivers — hourly, daytime, overnight, or weekend. We get to know the person before the first shift so families can rest, knowing the right hands are on duty.
Individualized services and supports that help people find and keep paid employment in community businesses and settings. We focus on real wages, real coworkers, and real growth.
